Eyelid Surgery

Excess skin from the upper or lower eyelid area is common with aging and makes a patient look “tired”. Occasionally, this skin may obstruct peripheral vision. This excess skin may be removed. This is called blepharoplasty.

Case 1: This patient shown here has excessive skin of the upper eyelids which required an upper blepharoplasty. She also has a “tired” appearance because of the “bags” under her eyes-which required a lower blepharoplasty. Spider veins of her nose were treated with laser. She also had fractional resurfacing of her sun-damaged skin.

Case 2: In this surgery, the extra fat needs to be trimmed off, as well as the extra skin. Recovery is in 2-3 weeks.

Case 3: In men, blepharoplasty is a little different than in women, because we try to avoid excessive tightness, which tends to give a more feminine look. Leaving a little skin overhang maintains a more masculine appearance.

Blepharorrhaphy

Blepharorrhaphy is the suturing of the eyelid to itself. This makes the eyelid fold over, onto itself.

Unlike blepharoplasty, blepharorrhaphy is reversible and has a much smaller scar. However, it is not well suited for patients with moderate to severe excess of eyelid skin or fat. It is offered to patients with little excess eyelid skin or fat.

Asian Upper Blepharoplasty

In Asia, the so-called “double-eyelid operation” is synonymous with upper eye lid lift (blepharoplasty). Anatomically, it is slightly different from the upper blepharoplasty performed in non-Asians (like case 1 above).

Case 4: Upper blepharoplasty in Asian American female.

Upper blepharoplasty, is the removal of skin and fat from the upper eyelid area. By removing excess fat and skin, there is less skin/fat overhanging the eye. Thus upper blepharoplasty makes the eye appear larger and more round. Recovery from a lower blepharoplasty is 2-3 weeks.

Sunken eyebrows create an “angry” appearance. An eyebrow lift corrects this. Sometimes blepharoplasty is combined with an eyebrow lift, as in this case.

Case 5: This patient had an eyebrow lift as part of his face lift. He also had an upper and lower blepharoplasty.

Notice how before the eyebrow lift, the patient has a more “angry” or “sunken” appearance.Canthoplasty is a procedure that makes the eye more round. It may be performed on the inner or outer angle (canthus) of the eye.

Sometimes all that is needed is the removal of excess eyelid fat, not blepharoplasty per se, which removes skin and fat.
